We Need Your Information
The Database Team works year round on adding , updating and verifying information of health and human services. The database is updated every day as we learn about changes. The nation’s oldest not-for-profit Family and Children's Service of the Capital Region is partnering with the Northeastern Regional Collaborative (NERC) in the development of the Human Service Info Line of the Northeast Call Center and we need your assistance.

Add/Update New Agency/Site/Programs
To make it as easy as possible, please use the simple instructions
INSTRUCTIONS
The profile is a Microsoft word form document that will need to be filled
out as completely as possible. Depending on the size of your Agency, you may
or may not have to fill out all of the sections on this profile.

Please open and save each of the forms to your computer.

Please specify when you save the forms if it is an Agency profile form (p-1), Site
-
Location profile form (p-2) and Service/Program profile form (p-3).

Once the profile has been saved on your computer, you can access it and save your changes at any time.

To save your changes click on file then click save.

Complete the “Agency” form (P-1). Please fill in every field – if the question is not applicable to your agency please put N/A.

Once you have completed the “Agency” form please save the information.

Copy and save the blank “Site Locations” form (p-2) as many times as necessary on your computer (according to the number of Site Locations that your Agency maintains).

Complete one form for each of your Site Locations.

Copy and save the blank “Service/Program” form (p-3) as many times as necessary on your computer (according to the number of Services/Programs that your Agency provides).

Once you have completed all of the information please save all of the profiles on your computer and then email to Submit
Inclusion and Exclusion Criteria:
Inclusion and exclusion criteria have been developed and approved by the governance of the NERC. The criteria include not-for-profit health and human services providers and for profit providers of services. The inclusion and exclusion criteria will be reviewed on a yearly basis by NERC and will be updated as needed. The criteria serve to guide and focus the data development process that can help to prioritize, through sequential order, which non-profit health and human service organizations will be entered into the database.
